Filling in forms electronically
When you have downloaded and opened the relevant form, you will need to click 'Enable Editing'. This will enable you to fill in the document electronically.
During COVID conditions, please make sure you email the relevant consent form to school prior to bringing in medication. Simply, download and complete the form electronically and then email it back to school (firstname.lastname@example.org). This is to reduce the opportunity of transmission. After completing the form, you can hand your medication to school staff at your child's classroom door. The support staff will check that this is for your child and you have completed a consent form.
When COVID conditions are removed, we will return to our Managing Medicines Policy and Procedures.
